Although they’ve been around for decades, bioidentical hormones are definitely getting more air time these days: from Suzanne Somer’s frank discussion on Larry King Live to chat rooms buzzing with questions about the benefits of bioidentical versus synthetic hormones, women like you are trying to get to the heart of a very complex question: are bioidentical hormones right for me?
Estimates suggest that about 75% of women through perimenopause will experience uncomfortable symptoms that prompt them to seek help. Many are able to manage their perimenopausal symptoms effectively through diet and exercise, relaxation and supplementation with vitamins, minerals or herbal products. Fortunately, when we take the time to carefully weigh the benefits of bioidentical versus synthetic hormones, we get encouraging results: • Since bioidentical hormones are identical in molecular structure to hormones that our bodies naturally produces, their effects are more consistent with our own hormones. Synthetic hormones, in contrast, are similar in some ways to naturally occurring hormones, but they aren’t identical. They may also contain elements that are not found at all in the human body. This causes them to interact differently with hormone receptors in our bodies and allows for a greater chance of undesirable side effects. In addition, when a synthetic hormone is metabolized, it may result in products that may cause further damage. For example, women who take Premarin, a synthetic estrogen, derived from the urine of pregnant mares, report having side effects like headaches, bloating and sore breasts. And there is some evidence to suggest that when metabolized, that Premarin can increase the risk of cancer in susceptible individuals. • The well-known 2002 Women’s Health Institute (WHI) study that found hormone therapy significantly increased the risk of heart disease, stroke, blood clots, breast cancer, Alzheimers and dementia, assessed women using synthetic hormones, not bioidentical progesterone or estrogen. • According to some studies, women respond more quickly to bioidentical versus synthetic hormones, finding relief from insomnia and hot flashes among other symptoms within a few days of beginning treatment. • Finally, unlike synthetic hormones, bioidentical hormones are not mass produced and can be tailored to your specific needs by a compounding pharmacist..

• Consider having your hormone levels tested first to determine which hormones may be beneficial. In women, hormone therapy typically focuses on estrogen, progesterone and to a lesser extent, testosterone; cortisol and DHEA may also be supplemented. Saliva testing looks at free hormone levels – hormones which are not bound to proteins and which your body can actually utilize. This is one advantage of saliva testing over conventional blood tests. Blood tests look at the total level of hormones, and not what your body is able to use. • Become familiar with the role of different hormones in your overall health and learn as much as you can about the merits of bioidentical versus synthetic hormones:
• Estrogen, for example, offers protection from a wide range of illnesses, including osteoporosis. It also boosts mental clarity, increases our sex drive and enhances our sleep among other things. Bioidentical forms of estrogen include estrone, estradiol and estriol and are available from a compounding pharmacy. Pre-made estradiol products are also available and include Estraderm and Climara (skin patches), Estrogel (topical gel) Estring (vaginal ring) • Progesterone decreases our risk of getting endometrial cancer and breast cancer; helps to build bone; enhances thyroid function; and has a calming effect. It also helps to balance the effects of estrogen. Many studies suggest that progesterone is best absorbed via the skin. It may be compounded by a specialty pharmacy or can be purchased over the counter as a 2% cream in the US. It is also available as the manufactured products Prometrium (oral capsule) and Crinone (vaginal cream). • Testosterone decreases fatigue; strengthens bone and boosts energy levels. Bioidentical testosterone is prescribed to many women struggling with low libido. • Be prepared to request bioidentical forms of any necessary hormones from your doctor. Despite the fact that they’ve been readily available on the market for more than 20 years, many physicians still routinely prescribe synthetic hormones over bioidentical ones. • When reviewing information on websites, even this one, remember to think critically about everything you read. It can be especially dangerous to diagnose yourself with an imbalance like estrogen dominance or adrenal fatigue and then self-prescribe hormones like bioidentical progesterone, which is available for sale on several US-based websites without a prescription. Just because you’re using a bioidentical product, it doesn’t necessarily come without the potential for side effects. Always seek the advice of a qualified professional. • If you are currently taking synthetic hormones, work closely with your health care provider to transition safely from synthetic to bioidentical hormone therapy. It can be done! • Discuss your test results with your healthcare provider and develop a treatment plan the addresses your unique needs.

Remember, it takes more than a tube of bioidentical progesterone cream to restore hormonal balance. Because hormones are part of a system, when one is out of balance, others may also be affected. And, bioidentical hormones, even those available without a prescription, are drugs. Most professionals will recommend the lowest dose for the shortest possible time. Patience is important as one-size-fits-all approaches won’t work and improvements can take time. Dosage adjustments are often needed before finding a regimen that works for you.
